In-home Care Case Management

The In-Home Care Case Management program provides culturally appropriate in-home care for ethnic Chinese elderly who are age 60 and above or disabled adults 18 and above. Case managers conduct interviews to assess the clients' needs, develop care plans, work in coordination with home care and service providers and monitor client progress.
