In-home Care Case Management
The In-Home Care Case Management program provides culturally appropriate in-home care for ethnic Chinese elderly who are age 60 and above or disabled adults 18 and above. As the front-door agency, CISC is responsible for the intake, assessment, care plan development and service implementation for ethnic Chinese who seek in-home care case management in King County. Case managers conduct interviews to assess the clients' needs, develop care plans, work in coordination with home care and service providers and monitor client progress. Our partnership with the Aging and Disability Service of City of Seattle enables us to help clients remain in their own home and community to avoid unnecessary and early institutionalization due to their disabilities.
We provide care for low-income individuals that:
- Need assistance with managing personal care and daily living tasks;
- Are homebound or physically isolated due to health reasons;
- Lack or have limited family support;
- Are frail and need cognitive support.
The CISC In-Home Care Case Management program is funded by the City of Seattle and all services are free of cost to our clients if they meet the state requirement of the program. Seniors 60 and above and disabled adults are encouraged to call or visit the CISC Main Office to enroll in the program Monday through Friday, 8:30 a.m. to noon, and 1:00 p.m. to 5:00 p.m.
