Taking care of and respect for our elders is a time-honored tradition. Our programs for older adults support their health and well-being. Our staff are committed to providing services and connecting seniors to resources that maintain and improve their quality of life.
Information & Assistance
CISC serves a vital role in improving the well-being of older adults. We listen carefully to the needs of older adults and work to connect them to a range of services to address social determinants of health. We have formed partnerships with funders to provide these services throughout King County to community members from various cultural backgrounds. Our team members are skilled in enabling clients to access programs including:

Community Living Connections (CLC)
CISC is a provider in the CLC network in King County. We provide short-term support and crisis intervention to help low-income older adults (55 and older) and adults with disabilities (18 or older) with no or limited English skills to access crucial services. In addition to information & assistance service, through outreach and collaboration with local service providers and partners, we engage and connect older adults in Seattle and King County with social and cultural activities to promote their physical, social and emotional well-being.

East King County Family Resource Center
We support immigrants and their families in accessing community resources and promoting their civic engagement through activities and workshops. Our Family Support Specialists provide professional bilingual and bicultural assistance such as information and assistance, education, outreach, and advocacy to help immigrant families access resources and thrive in the community. The program works closely with city governments in the Eastside area, including Bellevue, Redmond, Issaquah, Kirkland, and Bothell, as well as network partners, to address emerging needs that impact immigrant communities in areas such as affordable housing, financial stability, healthcare, education, and civic engagement.

Seattle Housing Authority (SHA) Case Management
We provide information and assistance, person-centered options counseling, care coordination, outreach activities, and case management for SHA residents, including Beacon Tower, International Terrace, Leschi House, and Gideon-Mathews Gardens, by maintaining a comprehensive and coordinated system of services. As a result, residents gain an enhanced quality of life and a stable living situation.

Senior Activities
We create a welcoming environment that honors the cultural heritage of the older adults we serve and enhances their well-being. Our programs promote health, emotional well-being, and life-long learning through activities and social support.
Sunshine Garden Chinese Senior Community Center: The Sunshine Garden, located at CISC’s Seattle Office, offers older adults culturally relevant activities that promote food and nutrition, health, wellness, and fitness, education, recreation, and socialization, and personal growth. This program is funded by the City of Seattle’s Human Services Department.

Russian-Speaking Senior Day Program: Participants enjoy a welcoming, supportive environment with access to resources and culturally-relevant programming. The program operates Tuesdays and Fridays at the North Bellevue Community Center. Our team members design activities that will promote health, social connections, well-being, and quality of life. This program is funded by the City of Bellevue and Amazon.

Case Management & Care Coordination
Older adults prefer staying in a familiar environment and in the community. Our dedicated team members enable seniors to maintain their independence by providing them with support, resources, and specialists.
In-Home Care Case Management: We provide linguistically and culturally appropriate case management services for Chinese and Russian-speaking vulnerable elderly and adults with disability. Case Managers complete comprehensive assessments to develop care plans tailored to individuals’ unmet and specific needs; and obtain and coordinate services to monitor their progress. The program enables vulnerable older populations and disabled adults to remain living in their own homes and communities safely and independently.

Available in:
Care Coordination Program: Offers short-term case management services to older adults and adults with disabilities in King County. These individuals need assistance accessing community-based resources to support their overall healthcare needs, especially those transitioning from the hospital back to the community. Eligible individuals include those aged 60+ or adults 18+ with disabilities who do not receive ALTSA-funded Long Term Care services. Our program offers comprehensive assessments, collaboratively creates personalized service plans, and conducts regular follow-ups to ensure effective support.

Care Transitions Program: A short-term case management program, we serve adults in King County who need assistance transitioning from the hospital to home and preventing rehospitalization. It includes a comprehensive assessment, service plan creation, and follow-up to adjust the plan as necessary. The program is available in person, over the phone, or through electronic communication and offers culturally appropriate and in-language support. There are no financial or citizenship requirements.

Caregiver Support
A time-honored tradition, family members and friends serve a vital role in taking care of loved ones.
Family Caregiver Support Program: We provide case management services for unpaid Chinese and Spanish-speaking caregivers who take care of their family members, relatives, or friends. Services include training and education, supportive counseling, referral, respite service, and long-term care planning. Our goal is to help caregivers adapt and adjust to the challenge of fulfilling the needs of their family members, relatives, or friends and prevent burnout caused by intensive caregiving.

Available in:
Medicaid Transformation Project (MTP): Washington State is offering two additional support services: Medicaid Alternative Care (MAC) and Tailored Supports for Older Adults (TSOA) to support older adults who need help to live at home. These programs provide timely, limited services and support to unpaid family caregivers or individuals without a caregiver. This program expands income eligibility for individuals who are at risk of needing long-term services and support.
